• Flower: White elongated cluster
• Leaves: single leaf, 1-3 inches long, up to 1½ inches wide, oval shaped, sheathes stem
• Fruit/Seed: oval capsule, 1/6 inch long
• Height: 4-9 inches
• Flowering: June - July
• Habitat: sphagnum hummocks in conifer swamps, peat soil in hardwood swamps
• Other: State Special Concern status
